• ​Eligibility: Children must live in your home and be verified prior to registration (e.g, medical card, birth certificate, social security card)
• ​Registration: Open now
• ​Intake: Email signup@prismmpls.org, register with a PRISM case manager in person, or register online and include the following information: Caregiver name, address, email, phone number, number of children, and children details (name, date of birth, grade, school district)
• ​Documents: Proof of address (e.g, lease, mail, utility bill); Proof of children’s residence (lease, school enrollment, financial documents, etc.)
• ​Distribution: Shopping appointment on Saturday, December 14th 9 am – 3 pm; Appointment confirmation required, you do not have an appointment unless you receive confirmation from staff
• ​Description: Parents shop for gifts for their children
